Note:  For SEAMIST senior missionaries as well as missionaries in training, this report is required to be filled in and sent during the first week of each month.  For other missionaries and workers assigned to or affiliated with SEAMIST, this report is optional.  The most important function of the report is to help the missionary evaluate his or her month.  The second most important function is to help SEAMIST leadership to know how best to serve the missionaries.  May God help you to see Him and what He is doing in your life more clearly!

Important instructions:  Regarding confidentiality and privacy.  Your report is to be sent by email to the SEAMIST General Director.  He may share parts of it with SEAMIST office staff for prayer or with your team leader to help them help you.  He will always send a copy of your entire report to your pastor.  You will be asked before anything in your report, including prayer requests, is shared with anyone other than SEAMIST staff, your pastor, and your team leader.  If there are any parts in your report that you do NOT want shared with anyone, be sure to indicate that part is "confidential" or "private" or "NOT for sharing".  Doing so will insure that only the Director and your pastor will see those parts of your report.  Your reports will be filed with any confidential parts blacked out.

How are you and your family doing? Areas for reporting are physical and emotional health, culture adjustments, language learning, your marriage, your family relationships, how your spouse and children are doing, the things you have learned about yourself and other family members.
How are you doing spiritually?  How is your relationship with the Lord?  Your devotional life?  Are you growing stronger or weaker?  Why?  What is one thing the Lord has shown you or done in your life this month?
What brought you the most joy this month?
What brought you the most frustration?

Your Team Life

How is your relationship to your team members going? Are you accomplishing your role on the team? Why or why not? What has brought you the most joy in the last month? What brought you the most frustration? What team plans do you think are moving ahead? Which plans are not working well? What do you think should be different about the team? How can the other members of the team help you? What can you do more to help the team? What suggestions do you have for changes?

Are you happy with the relationship you have with your team leader or overseer? Why or why not? What can they do to help you more during this coming month? What do you wish they would do more of? Less of? What one suggestion would you make to them that would make their ministry to the team more effective? What things do you feel must change so that the team can be more successful?
Your Ministry

You stated your mission and vision in the first part. What did you do last month to fulfill the vision? What progress was made in completing your projects and plans last month? What ministry projects or plans were accomplished? What ministry activities do you plan to do next month? How can your team or your leader help you? What would you like to change about your ministry?
Your Support and Supporters
For SEAMIST workers, optional for others!

In what ways did you contact your supporters this month? How? Did your pastor contact you? How? Did any of your supporters contact you? How? What can you do to involve your supporters more next month? How could your supporters help you more?
How much income did you receive this month in total?  Was it enough to meet your needs?  Enough to minister?  How are you doing financially?
